Located in Heidelberg and a 4 minute drive from Northland, this property consists of a two bedroom high physical support unit and a two bedroom villa.
Each dwelling has an attached OOA and the bedrooms in the villa each have a private courtyard.
Care is delivered by a supportive team with the option to choose all female staffing so you can live your best life. Participants are able to bring their own community access workers, whether independent or with other providers for any supports they require outside the home.
We can work with you on your timelines, supporting a rapid entry if you are urgently needing accommodation or taking it slower to fully plan around your needs and goals with a staged transition into the home.
Home features include fully accessible ensuite and bathroom attached to each bedroom, high accessible design throughout including kitchens and living areas designed to maximise opportunities to participate in daily activities. There are no steps throughout any parts of the home.
Common areas are fully furnished with Netflix accounts on the TVs provided. The homes have full backup power generation and interconnected fire systems linked to local fire brigades. All bedrooms are spacious with built in robes and ample opportunity make the room truly yours.
This property is available to NDIS funded participants who are SDA funded. As a currently vacant property we are focussed on strong matching between residents and can accommodate people looking to move in as either single tenants or as a pair of residents.
Via Care will deliver the SIL supports within the home. Via Care has a long history of specialising in supporting people with high physical and medical needs. All our staff are medication and diabetes educated and can be trained in further specific needs.
Our team have strong social work backgrounds in addition to long standing disability and medical experience. This means our care is informed by social work principles and case planning. We strive to maximise empowerment, quality of life and social connection and regularly find innovative ways to build social participation opportunities for participant within the home and their local community.
